CAP’s High-Needs Initiatives Team hosted this class to discuss the need for and mechanics of transition planning for youth with significant mental health concerns, intellectual disabilities, and/or developmental disabilities as they transition between levels of care and placements. These youth require more than a bed; therapy, educational support, and in-home services may also be needed.
